The College of Nurses of Ontario hosted several provincial DNR teleconferences in the fall 2007 and winter 2008 to introduce the new Do Not Resuscitate Confirmation Forms.  The Network facilitated sites across the HNHB area for health care providers to participate in these teleconferences. The Network also offered Fire Side Chats through the SHRTN EOL Community of Practice in early January. Additional in-service education sessions can be arranged by contacting the Network.

The DNR Confirmation form can be viewed online at the MOHLTC website but it can not be printed or completed online as each form has a unique serial number.
