Seniors Health ResearchTransfer Network

In the Seniors Health Research Transfer Network (SHRTN), a Community of Practice (CoP) is a group of people who come together to exchange information and knowledge on a specific topic related to seniors' health and health care. Please visit the SHRTN website at http://www.SHRTN.on.ca.

The End-of-Life Care (CoP) is one of SHRTN's 19 Communities of Practice. The purpose of the CoPs is to provide hospice palliative care providers with the mechanisms to work in collaboration. HNHB HPC Network members would find participation in the End-of-Life Care CoP valuable. To view the End-of-Life Care CoP brochure
