As per recent policy changes from the Ministry of Health and Long-Term Care, the new HNHB HPC Network will act in an advisory capacity in planning, delivery, implementation and evaluation of the Palliative Pain and Symptom Management Consultation (code 24A) service, and Facility Palliative Care Interdisciplinary Education (code 23A) service. 

The Palliative Care Consultant (PCC) is a position funded through the Ministry of Health and Long-Term Care. PCCs facilitate palliative care education, consultation and linkages for staff of Long-Term Care Homes, Community Nursing and Support Agencies, Hospices, Day Programs, Supportive Housing and other organizations. The Palliative Care Consultant serves as a resource to assist staff in the interpretation and application of The Model to Guide Hospice Palliative Care, best practice guidelines and assessment tools. The services provided include telephone and on-site consultation for care providers, problem based education, training and mentorship for front-line staff, and information on current, research based best practice guidelines related to palliative care and linkages to other palliative care resources. PCCs provide consultations to staff. They do not provide direct care.

The Ministry of Health and Long-Term Care recognizes that synergies exist between the PPSMC service and the Palliative Care Interdisciplinary Education service, and in regions where the administration for these services has been aligned/consolidated, significant benefits have been noted. This resource (PPCs) is available within the Haldimand Norfolk and Brant, Burlington, Hamilton and Niagara areas.
