A couple of weeks ago at shift change report a family doctor came by to see his patient. I updated him on his patient's condition which included some difficulty swallowing oral meds. I reminded him to write the orders for the med changes. At this point I sensed his discomfort as he sat looking at the blank order page. He asked me what he should order in a very quiet voice so that only I could hear him. This clearly was my moment!!!! Feeling very confident I reached into my CAPCE bag and pulled out my opioid rotation sheet. Following the steps I calculated the new dose for s/c administration including the BT dose. The doctor was very impressed and actually requested a copy of my handy dandy little sheet. After the doctor left my co workers expressed interest in learning how to do the calculations themselves.

Wanted to let you know this course is paying off in a big way!! Today I had a patient unexpectedly take a turn for the worse. Without explaining everything, the patient wanted to stay at home and be kept comfortable so among other things I had to make arrangements for oral medications to be converted to S/C. I thought I had it made because a palliative care physician is involved. He was out of town!!! (see more)
I ended up having to sort it out with a F.P. covering for the regular F.P. I was able to figure out the conversion myself before the call and talk the physician through the method of conversion, (he asked me if there was "some sort of table to figure it out”). The math matched mine but I was a bit nervous. I was thinking of calling the Palliative Care Consultant when the palliative care physician called me back and went through the numbers again and I was right!!! What a confidence booster!!! Thanks.

Comprehensive Advanced Hospice Palliative Care Education Program for Nurses - CAPCE

To build hospice palliative care (HPC) capacity and expertise in the Hamilton Niagara Haldimand Brant (HNHB) area, the HNHB Hospice Palliative Care (HPC) Network with the support of the regional Palliative Care Consultants launched Comprehensive Advanced Palliative Care Education (CAPCE) – an advanced level HPC education program specific for nurses (RN, RN(EC) and RPN) in 2009.

The CAPCE Program, which was first developed and implemented several years ago in the Southwest region of Ontario, has now become widely adopted across the province as best practice evidence-based HPC education program.   The program focuses on developing a Hospice Palliative Care Resource Nurse within the health care provider organization in which they work – long-term care homes, hospices, hospitals, CCACs and community nursing agencies. 

This program is excellent for nurses who have a keen desire to grow their HPC skills and who have the interest and ability to become agents of change within their organizations.  Because the day-long, formal sessions of the course run only once a month for six months, the format is perfectly tailored to the needs of individuals who are attempting to balance their busy careers and home lives.

Program Overview: Next Course starts September 2011  Registration is now closed for 2011-2012

1.      The program includes six full-day sessions of interactive learning that occur at one month intervals between:

Start Date:  September 14, 2011                  Completion date:  June 2012 

 

To qualify for a certificate of completion attendance at each session is mandatory.  

There are no exceptions!

 

2.      There are no exams; however, there are several mandatory components:

 

þ     Pre-reading prior to each session.

þ     On-line learner reflection forms to be completed after each session.

þ     Practical application assignments that include documented evidence of completion of clinical assessments and written reflections on the application of knowledge to clinical practice.  The mentor will provide individualized feedback to each learner related to the assignments.

þ     A project specific to your work setting; a template for project submission will be provided.

3.      The local Palliative Care Consultant mentor will provide support to the learner throughout the course via phone, email, or face-to–face meetings.

4.      The mentor will provide senior management and the learner with support for the development and implementation of a project plan within each participating organization.

5.      Eligible learners are nurses who live and/or work within the HNHB region.

6.      What is the cost?  The CAPCE Investment is  $375 and includes:

·       6 sessions (lunch and refreshment breaks included):

·       Mentor support throughout the course and consultation thereafter

·       CAPCE Resource Guide 

·      Online support and additional learning resources
