Our Roles and Responsibilities

Network Role
To develop and ensure the implementation of strategic plans and policies for a system-wide approach to the delivery of palliative care services, including: education, research, advocacy, quality management and communication activities related to palliative care.

To coordinate and integrate palliative care services within HNHB by:
·      Establishing criteria for prioritizing the activities to be undertaken by the NETWORK
·      Maintaining and fostering linkages with relevant organizations, physicians, consumers and families, academia and government departments
·      Working in partnership to coordinate, integrate and improve the system of hospice/palliative care
·      Collaborating on funding proposals involving palliative care service, education and research initiatives.
 
 To develop quality management activities by:
·      Identifying, documenting and promoting best practices in the delivery and standards of palliative care services
·      Coordinating activities to improve the efficiency and effectiveness of the system for clients and their families, i.e. the development of common assessment tools, intake forms and a database
·      Identifying and evaluating program outcomes.
 
To provide a forum for receiving and responding to MOHLTC policy initiatives and for participating in the review and development of documents related to palliative care.
 
To identify gaps in palliative services within HNHB.
 
To develop process mechanisms for resolution of issues across disciplines and organizational boundaries, related to the implementation of standards, guidelines and other initiatives.

To establish a mechanism to oversee operations, education, evaluation, policies and procedures, and communications.

Network Responsibilities

Network Members
The HNHB Hospice Palliative Care Membership is responsible for demonstrating a willingness and ability to work together with the Network constituency towards common goals in order to improve hospice palliative care within and across the HNHB system of care.

Network Advisory Committee
The HNHB Hospice Palliative Care Network Advisory Committee is responsible for developing and recommending system-wide strategies to respond to key priority issues, and Network operational decision making.

Network Community Committee
The HNHB Hospice Palliative Care Community Committees are responsible for collecting and communicating local input and feedback to the Advisory Committee, disseminating information from the Advisory Committee and Council to the local communities, and implementing at the community level system-wide strategies to respond to key priority issues.

Network Council

The HNHB Hospice Palliative Care Network Council is responsible for system-wide governance decision making for HNHB hospice palliative care.
